Bring a Patient
If you have a patient you would like to bring to a meeting for discussion, the following is an outline of helpful steps to make it easy for you:
1.Print map/directions for patient from links below. If location is different than UT Southwestern or Dr. Giles, you will need to personally give patient directions.
2.Write a protocol on your letterhead that outlines the problem, exam, diagnosis, etc. Make copies and bring to the meeting.
3.Contact Cockerell & Associates if they did the path so the patient's slide can be shown at the meeting for clinicopathologic correlation. If you use another service, it would be helpful to get the slide sent to Cockerell or have them email you the jpeg images of the pathology sections. Would be helpful if Cockerell could have at least 24 hours notice.
4.If you are able, please have patient and yourself sign consent on the day of the meeting. For copy of consent, click on link below. Give consent to an officer to be filed. Consent forms will be available at the meeting, ask an officer.
5.Meet patient at the meeting start time if at all possible. It is not appropriate to have a patient come unattended by their physician unless other arrangements have be made for another physician to sponsor the patient.
